Grief is often discussed through decisions and practical planning, yet conversations about identity, values, and relationships are equally important and overlooked.
This session introduces the Life Certificate grounded in Narrative Therapy, inviting participants to document cherished stories, significant relationships, hopes and preferrred legacies. Through experiential reflection, participants will discover how remembering conversations can strengthen connections and create meaningful keepsakes for their losses.

Lew Xian Xiong
Narrative Therapy Practitioner / Senior Medical Social Worker
HCA Hospice
Lew Xian Xiong is a Narrative Therapy practitioner with a Master of Narrative Therapy and Community Work from the University of Melbourne in partnership with the Dulwich Centre and a Registered Medical Social Worker.
He practises in hospice care, supporting individuals and families experiencing life-limiting illnesses, grief and bereavement. His interests include adapting narrative practices for healthcare, with a focus on strengthening identity, preserving legacy and creating meaningful conversations at the end of life.
He is currently developing the Life Certificate as an innovative narrative document for palliative care and bereavement support.
